CNA/HHA-Home Health

We are seeking a compassionate and reliable CNA/HHA to provide high-quality, individualized care to clients in their homes. The ideal candidate will assist patients with activities of daily living (ADLs), support their health and well-being, and promote independence and comfort in a home setting. This role is essential in helping clients maintain dignity, safety, and quality of life.

• Assist clients with personal care needs, including bathing, dressing, grooming, toileting, and oral hygiene.

• Provide mobility assistance, including transferring, positioning, and ambulation.

• Assist with meal preparation and feeding as needed.

• Perform light housekeeping duties to maintain a safe and clean environment.

• Monitor and report changes in client condition to supervising nurse or care manager.

• Follow the care plan developed by the RN or case manager.

• Assist with medication reminders (if permitted by state regulations).

• Offer companionship and emotional support to clients and their families.

• Maintain accurate and timely documentation of services provided.

• Adhere to all infection control, safety, and confidentiality policies and procedures.

• CNA or HHA certification in the state of Ohio.

• Current CPR certification.

• At least [6 months – 1 year] of experience in a home health or long-term care setting (preferred).

• Ability to work independently and manage time effectively.

• Strong communication and interpersonal skills.

• Reliable transportation and valid driver’s license (if travel is required).

• Ability to lift, push, or pull up to 50 lbs.

• Ability to stand, walk, bend, and kneel for extended periods.

• Must be able to handle the physical and emotional demands of caregiving.
